There are very few things I'm more confident saying today than this: If you've been diagnosed with Behçet's disease, I believe there's an excellent chance we can help you. Twenty years ago, I never would have said that because twenty years ago, I didn't know.
That’s when I saw my first Behçet's patient, and she wasn't looking for someone to treat her Behçet's disease. Her name was Gretchen, and she came to me because she had terrible digestive problems.
She already had the Behçet's diagnosis. She had already seen the specialists. She had already taken the prednisone, the methotrexate, the azathioprine, and the biologic drugs. She still had painful mouth ulcers, painful vaginal sores, joint pain, muscle aches, crushing fatigue, and now IBS on top of everything else.
When she came to see me, I wasn't trying to solve Behçet's disease. I was trying to solve her digestive problems. That's all.
So we did what I've always done. We looked for the underlying problems instead of simply accepting another diagnosis and another prescription.
Her digestive symptoms improved. Then something happened that I wasn't expecting. Her mouth ulcers disappeared. Her vaginal sores disappeared. Her joint pain disappeared. Her fatigue disappeared. She eventually came off every medication she was taking for Behçet's disease.
I remember thinking there were only two possibilities. Either we had just witnessed an extraordinary coincidence... or we had discovered something important.
At first, I assumed it was coincidence. Then another Behçet's patient came. The same thing happened. Then another. The same thing happened again. Eventually, the evidence became impossible for me to ignore.

That's why I no longer think of Behçet's disease the way I once did.
Most physicians are trying to control Behçet's disease. We're trying to understand why it's happening. Those are very different goals.
People often ask me, "So what's the cause?" I don't know, because it’s different in every patient. And anyone who tells you there's one cause for everyone isn't being honest.
What I do know is this: When we identify and treat the factors driving the illness in that individual patient, remarkable things happen. I've watched it happen repeatedly.
Gretchen later wrote this:
"Within two weeks, my energy doubled. Getting out of bed wasn't agonizing anymore. A few months later I was down to one medication—from eight. The things that used to be impossible—traveling and having kids—are now a reality."
That's incredible. That's her life.
Now I realize many people won't believe me. I understand. If someone had told me this before I started seeing it with my own patients, I wouldn’t have believed it either.
But I can't ignore what I've witnessed over nearly two decades. And I'm confident enough to stand behind it.
